学生奖励申请表格:

All student requests for funding require a budget. 请下载适当的 下面的形式, and upload it into your application.

All project award applications require the submission of a project timeline. 

​All students applying for funding must have their mentor complete the appropriate 获奖表格. 
The deadline to submit this 形式 is the same as for the awards.

This agreement establishes the expectations for undergraduate students and their UAF mentor as a guide for the student's research and/or scholarly activity experience within projects funded by the Office of Undergraduate Research and Scholarly Activity (小熊星座). This agreement 形式 the basis for assessment of progress and evaluation of outcomes, either formally or informally, and to ensure that the needs of the student and mentor are being met during the project period. 

使用方法: 学生和导师 should complete this agreement together using the template to facilitate your conversation as you establish and clarify expectations for communications, 里程碑和结果. Fill it out at the beginning of the project and refer to it as needed during the project term. Use the agreement to (1) discuss and reflect on the student’s progress, (2) ensure that the mentor is meeting the student’s needs, and (3) assess the mentee-mentor experience and relationship. 学生和导师 should retain a copy of the agreement throughout the term of their project.

All students receiving 大熊星座 awards are required to complete the a reflection form (evaluation) before the end of the semester when they received their awards.

All students receiving 大熊星座 awards are required to complete the a reflection form (evaluation) before the end of the semester when they received their awards:

  • AY 2022-2023截止日期
    • 2023年夏天: 8月. 11, 2023
  • AY 2023-2024截止日期
    • 2023年秋季: 2023年12月10日 
    • 2024年春季: 2024年4月28日
    • 2024年夏天: 2024年8月11日

All students receiving 大熊星座 awards are required to complete the a reflection form (evaluation) at the end of their award period:

  • Reflections for this award will be due within two weeks of the completion of the internship. 
    • Specific reflection due dates will be communicated in the award paperwork upon awarding.

    • Evaluations must be completed at the midterm and end of an awarded opportunity.

All Mentor Award recipients are required to complete the reflection form (evaluation) before the end of the semester when they received their awards. 

All CEL Award recipients are required to complete the reflection form (evaluation) 下面. 

All recipients of 大熊星座 ITE awards are required to complete the reflection 下面的形式
